I spoke to someone from customer service last week in regards to a transaction that neither myself on my wife did, where do I find the form to fill out to dispute this transaction and where do I send to.

Hi @mifsudds, I'm sorry to hear you're needing to dispute a transaction.
You can obtain the form using this link - under the general forms heading. Once completed, you can forward this through BankMail, via the "Message Centre" tab of your Online Banking. It will be received by our email team, and responded to within 1 - 2 business days.
Alternatively, you can either fax the form to 1300 765 515, post it to PO Box E237, Perth WA 6841, or you can drop it into your closest Bankwest store.
